Sadness..

The photo is extremely touching and produces a strong effect upon the viewer. There is this strong contrast between the focused, left area and the unfocused, blurry right area which immediately draws attention. The high level of details provided together with the black-white profile suggest just this idea: the sadness and the worries of the old age.

Regarding the composition, I really love the angle from which the shot was taken. It gives the photo an air of "spontaneous photo" but not quite.
As a small suggestion: I think it would be nice to capture the whole model.
However, the open mouth, the position of the eyes and all the wrinkles let one pass over such minor details <img src="e.deviantart.net/emoticons/w/w…" width="15" height="15" alt="" title=" (Wink)"/>
